ProCare Oregon


 

ProCare Oregon is an individual dental insurance plan certified to provide coverage of the pediatric dental essential health benefit, in addition to adult dental coverage and orthodontia coverage for all ages.

There are two ProCare Oregon plans available – Plan 1 and Plan 2. You can purchase a ProCare Oregon plan through HealthCare.gov as well as directly from Willamette Dental Group at the same rates and with the same benefits.

To Apply

Through HealthCare.gov:

Open Enrollment through the Marketplace for coverage in 2017 ended on January 31, 2017. Open Enrollment through the Marketplace for coverage in 2018 begins November 1, 2017 and goes through December 15, 2017.

To enroll through the Marketplace, visit HealthCare.gov.

If your application is completed and submitted through HealthCare.gov by the 15th of the month, generally, your coverage will be effective on the first of the following month.

Direct with Willamette Dental Group:

To enroll in one of the ProCare Oregon plans direct with Willamette Dental Group, simply complete both sides of the application form above, including your plan selection and signature on the back, and submit it along with your premium payment.

If the application and premium payment is received by the 25th of the month, your coverage will be effective on the first of the following month.

Who is Eligible?

You are eligible to enroll in ProCare Oregon plans if you are an Oregon resident living within our service area. When enrolling through HealthCare.gov, the Marketplace will determine you and your dependents' eligibility for coverage. Your eligible dependents may include your spouse or domestic partner, child, and spouse's or domestic partner's child.

Our services area includes the following counties: Benton, Clackamas, Columbia, Deschutes, Douglas, Jackson, Jefferson, Josephine, Lane, Lincoln, Linn, Marion, Multnomah, Polk, Tillamook, Washington, and Yamhill.

The following counties are not within our service area: Baker, Clatsop, Coos, Crook, Curry, Gilliam, Grant, Harney, Hood River, Klamath, Lake, Malheur, Morrow, Sherman, Umatilla, Union, Wallowa, Wasco, and Wheeler.

Members may not be enrolled under any other insurance policy or contract issued or offered by Willamette Dental Group or its affiliates. If you enroll in a ProCare Oregon plan, your other Willamette Dental Group policy will be terminated.

How are premium payments made for ProCare Oregon?

Through HealthCare.gov

After you have submitted your application for ProCare Oregon through the Marketplace, please wait to receive your premium billing statement to make your first payment.

You have three payment method options:

  1. Via Auto Pay – You may elect to have your premiums deducted from a checking account each month. If paying by AutoPay, please call 1-855-998-2273 to setup your payments.
  2. Via Credit Card – You may elect to have your premiums charged each month to a credit card. We accept Visa, Mastercard and Discover. If paying by credit card, please call 1-855-998-2273 to process your payment or to set up an auto-withdrawal for each month's payment.
  3. Via Check – If you are paying by check, enclose your check with the stub from your premium billing statement and mail to the address below:

Your first premium payment should be submitted with your application. You have three payment method options:

Can I drop-off my application and premium payment?

Premium payments are accepted via the methods listed above. Please do not remit payment to your Willamette Dental Group dental office. The dental office cannot accept payments from patients for insurance premiums.

Can I sign up for ProCare Oregon and still go to my own dentist?

To receive the excellent benefits of ProCare Oregon, you must receive care from a Willamette Dental Group dentist or specialist. Your coverage also extends if you are referred to an outside dentist or specialist by your Willamette Dental Group dentist. If referred to an outside dentist or specialist, your copayments remain the same as shown in your Summary of Benefits.

How do I schedule an appointment?

Monday – Friday from 7am – 6pm PT

What can I expect at my first visit?

During your first visit to our office, you will receive a thorough dental examination that includes X-rays and comprehensive risk assessments. Your dentist will develop a Proactive Dental Care Plan based upon your immediate needs, current dental health and long-term oral health goals. This individual plan will include recommendations for cleanings, restorations and preventive treatments. Most patients will receive a cleaning at their first visit, based on the assessment and recommendation from your dentist.

When can I start using my ProCare Oregon coverage?

Through HealthCare.gov:

The Marketplace will determine your coverage effective date. The Marketplace will provide us with your coverage effective date after the Marketplace has completed processing your enrollment application. To confirm your coverage effective date, please contact 1-855-998-2273.

Direct with Willamette Dental Group:

If the application and premium payment is received by the 25th of the month, your coverage will be effective on the first of the following month.

How do I know how much Willamette Dental Group charges for dental procedures?

After we receive your payment of premium for your ProCare Oregon plan, you will be mailed a printed copy of the plan policy. Please refer to this policy for a list of copayments for covered dental procedures. The costs of many dental services are covered with a reasonable service copayment.

Are there benefit waiting periods under these plans?

Under both ProCare plans, there is a 6-month waiting period to benefits for permanent crowns, bridges and dentures for enrollees age 19 and over. A 6-month waiting period applies to benefits for orthodontia treatment for all ages (unless for treatment of cleft lip and/or cleft palate in an Enrollee under age 19).

How do I cancel my ProCare Oregon policy?

Through HealthCare.gov

If you enrolled through HealthCare.gov, you must contact the Marketplace directly to cancel coverage. To request changes to your coverage (such as changing or terminating plans, adding/removing dependents), please call the Marketplace directly at 1-800-318-2596 (TTY 1-855-889-4325 or 711).

Direct with Willamette Dental Group

Change requests, such as adding/deleting a dependent, name and address changes, and cancellation requests, must be made writing. To make a change to your ProCare Oregon policy, please fill out this form and submit via mail, email or fax as noted below. The completed form should be sent at least 30 days in advance of your desired effective date. If you are canceling your policy, your coverage will terminate on the last day of the month following our receipt of your cancellation notice, unless your premiums are past due or you specify a later date.

Please note that if you cancel your policy, you must wait 12 months before you are eligible to re-enroll in any individual plan offered by Willamette Dental Group.
